Level 4: The Pit, The Dog and The Button
A downloadable game
You enter the room. There is single light coming from above but it’s impossible to say where from exactly.
In the middle of the room is a pit. Drop a coin and you won’t hear it hit the floor.
On the right there is a dog. Think twice before engaging him in a conversation.
Behind the dog is a button. You can try and press it.
There is another door on the other side of the room.

This is cruel torture. Players will be stuck here for hours looking for the solution.
That is exactly the point. Very much in spirit of the game! ;)
Do keep in mind it is meant to be a 5 page format. I think that you should SPOILER WARNING
Include 4 pages worth of stuff for DM giving advice on what to say and how to say it and some pre written stuff aswell. Stuff like "Please jump in the hole its how you will survive the trials ahead in the dungeon! Pleaseee i've been a very good boy patiently waiting so I can advise adventurers just like you as ive been asked by my owner" and add for them to describe how they start to do puppy dog eyes and whimper until eventually they become more angry deliberately mean. Have the dog call them foul things and insist that they are what they call the PCs because they want him to be punished for being a bad boy when their owner gets back.
Make it clear in the beginning that PCs are not allowed to describe what happens to them when they jump in unless they could reasonably do so. Like if they fell to the last floor and crawled their way up or if they made a strong enough strength check to yell loud enough while falling infinitely and give a presence check for the people who haven't jumped yet to hear it.
Include notes for how the dog will react based on what happens when they jump in and what the salvation entails.(maybe salvation is something like they pop out into medieval Europe
or the beginning of skyrim.Maybe the dog will say that he's so sorry and that the switch needed to be flipped the other way when they jumped down if a person crawls back to the room. Maybe make the dog say "Shit" and then attack them if the hole is really shallow.Some great advice here, but just to correct one thing - it's meant to be a A5 format, not "a 5 page format". So one-two pages is more than ok ;)
